    • Beneficiary Information (DKY04, 1 Page)
    • Release of Liability (DKY05, 1 Page)
    • Health Certificate (DKY06, 1 Page) (any medical doctor can sign it)
    • After you complete above forms,  use Fax/Mail Cover Page (DKY02, 1 page).

        3) Submit Passport to Mission Answers

  • Submit Passport to Mission (Self-training Mission Course)  You are exempted if you took Preparations for Mission class at one of the Adventist Colleges/Universities, in North American Division (NAD)
    • Off-line
      • Call 1.877.257.4086 (or email: info@sdakorea.org), ask to borrow Passport to Mission book
      • Read the book and answer Study Questions
      • After you wrote answers, you submit them to Institute of World Mission (IWM)
        • Fax to 269.471.6252
